Our Location:

We are located at 13410 Main Street in downtown Grabill, Indiana, one block south of Grabill Hardware. Our location is accessible to those living in northeast Indiana and northwest Ohio, and just 15 minutes from I69 & Dupont Road!

Public Transportation:

Transportation is available through CTN for any CCHC services during the hours of 7:00am-6:00pm. Patients must register and schedule transportation in advance of the Clinic session. Fees are based on household income. Visit the CTN website to register. Call 260-420-3280 to schedule a ride.
